Varying lengths of threads are used to reposition tissue across the midface, and the cones that are affixed around the thread hold tissue in place.

Treatment is conducted with a local anesthetic. Patients do not feel pain during their procedure. They may feel pressure and tugging as threads are inserted, which can feel somewhat strange, though not painful. After the threads are inserted, the tissue is gently pressed in an upward, outward motion. As this is done, the cones on the thread adhere to deeper tissue, locking it in place.

In addition to immediately repositioning tissue, an InstaLift also provokes a healing cascade in which collagen production increases for several weeks. Over time, as the sugar-molecule threads dissolve, tissue gains support from healthy new collagen fibers that continue providing firmness and suppleness.
